Now you can join our low-cost dental coverage for a nominal membership fee. As a member, Preventive Treatments (General Cleaning*, X-rays, Exams) are always covered 100%. The membership allows individuals and families to receive quality dentistry at prices 15 -40% off average dental treatment fees in the area.

Payment Policy

In order to provide the membership discounts to the treatment fees, the payment policies listed below must be followed:

• The annual membership fee must be paid in full at the time of enrollment.
• Membership period will begin on the date of payment of either initial payment or renewal and will be valid for one year.
• All fees for treatment will be due and payable at the time of service.
• DentalHealth360 reserves the right to terminate the membership and/or refuse treatment without notice if the member fails to pay any dues or their account becomes delinquent at any time.
